Model consent & release form.

I, the undersigned, hereby grant Trevor Reid Designs LLC, the irrevocable right to use, publish, and distribute my likeness, voice, and any photographs, video recordings, or other media of me, taken during the shoot, in connection with marketing, promotional, or advertising purposes. This includes but is not limited to use on websites, social media, print materials, and digital campaigns.*

I understand that these materials may be edited, altered, and used in any form and manner, in perpetuity, and waive any right to inspect or approve the finished product. I acknowledge that I will receive no monetary compensation for the use of my likeness in any media or marketing materials.*

I hereby release Trevor Reid Designs LLC and the Client from any claims, demands, or liabilities that may arise from the use of such photographs, videos, or other materials.
